New patients
Your first visit
What to expect, and what to bring.
You do not need a referral to be seen. Many patients come on their own, and many come because their doctor asked us to take a look.
What happens
- 01
Step 1: Book
Pick a time online, or call. If a doctor referred you, say so and we ask for the referral.
- 02
Step 2: Forms
Fill in the new patient forms before you come. The link is below, and paper forms are at the desk.
- 03
Step 3: The visit
Plan on about an hour, longer if your eyes are dilated. The doctor explains each finding.
- 04
Step 4: After
A copy of your prescription, and a report to the doctor who referred you, with your permission.
Billing and what to bring
How it is billed
A visit for an eye problem, an eye disease check or a follow-up is billed to medical insurance. A routine exam is billed to a vision plan. Many visits mix both, and we tell you which part goes where before you leave. If you pay yourself, we give you a written estimate on request.
How fees workWhat to bring
Current glasses and contact lenses
Vision and medical insurance cards
A list of your eye drops and medicines
